Voices & Choices Person-Centered Planning Project

Central to person-centered planning is a circle of friends and supporters, including the person with a disability, family members, friends, peers, teachers, and other service providers. In the PATH process this team of support creates a shared dream of a full and rich life for the person with a disability.  This method results in a strong commitment on the part of the team members to take specific actions now to help the individual progress along his\her path to the future.

Academic Course with Community-Based Outcome

A newly-revised curriculum for Human Services 240, "Person Centered Planning" and Collaborative Team Building has been implemented and the course is offered twice annually.  Students achieve competency in the knowledge of systems of care and associated resources, person centered planning tools and processes, facilitations skills, team building processes and skills, and person centered planning strategies in wraparound service planning.

Information & Referral Network

The vision of the project includes creating the “Voices and Choices Network,” a single access point for coordination of person centered planning services.  With the State of Alaska, structures will be provided to build a network of certified plan facilitators to support and manage the referral process, matching consumers with facilitators.

Outcomes

Using the Wraparound Fidelity Index, version 4 (WFI-4), process and outcome measures will be evaluated to assess level of consumer and family satisfaction with the person centered planning process.  The project should show that those who participate will experience greater satisfaction with their service plan and improved functioning in at least one life domain.
