INTRODUCTION TO TRANSITION

The decision of what to do after high school is difficult for anyone. For individuals with disabilities, the process may even be more difficult.

Transition

The word “transition” is used to describe the passage from school to adult life, from the classroom to the workplace. The goal of transition is to insure that assistance begins in school and continues afterwards to move the student gradually into employment and independent living to the greatest extent possible. This assistance will enable the individual to lead a more fulfilled life.

In the process, the responsibility for transitional activities begins to shift from school the student, the family, and service providers. During this period the student becomes more involved in the life of the community. With help from parents, families, caregivers and the support of educators and social service agencies, individuals with disabilities can effectively make this transition.

Transition and the IEP 

Successful transition requires careful planning, the full use of available resources, and a commitment of everyone who is involved. The Individualized Education Plan (IEP) must include a Transition Plan beginning at age 16. This planning has been mandated by federal law under the Individuals with Disabilities Education Improvement Act (IDEIA 2004).

IDEA defines transition services as a coordinated set of activities for a student with a disability that:

(A) is designed within an outcome-oriented process, which promotes movement from school to post-school activities, including post-secondary education, vocational training, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation;

(B) is based upon the individual student’s needs, taking into account the student’s preferences and interests; and

(C ) includes instruction, related services, community experiences, the development of employment and other post-school adult living objectives, and, when appropriate, acquisition of daily living skills and functional vocational evaluation
